CD Japan states that all the above are taken from night one of the Metal Weekend shows. But the last track of the Blu-ray (Loudness) was not played on night one, so either the setlist at setlist.fm was incomplete or they included that from night two or night four (or there is always a chance that the music for that song is just playing during the closing credits). In Japan there is a 1 Blu-ray + 2 CD version OR a 1 DVD + 2 CD version. Looks like the content between the Blu-ray and DVD are identical. Note that the Blu-ray will be region free, the DVD is region 2.

Currently, EarMusic is listing no other "format" than a DVD + 2 CD version. Their website doesn't specify what songs come from what shows. But it looks like the DVD is a mix of several different nights, CD1 is part of the night two show and CD2 is part of the night three show.
Looks like I might have to get both versions, although the Japanese Blu-ray + 2 CD version is really expensive ($72.64 with the current exchange rate).
As I'm cranking up Heavy Metal Hippies right now, I was curious for your thoughts on the Mike Vescera-era (God-awful to me) and the Masaki years (a lot of killer stuff and I found him to be a better singer than Niihara, but too much of it had a Grunge feel as the 90s went on).
Niihara is an acquired taste, but I love the guy and he really is an important part of their sound.
