Apparently, if you pre-order the album on CD or vinyl from Amazon UK, you get the EP for free. Since there's nothing exclusive on the EP I'm not sure what the benefit of that is, other than possibly getting a physical copy of three songs before the album comes out (depending on when they choose to ship it).

When is the U.S. version of this CD due out? I still only see the import version listed on Amazon. They have the U.S. version available for 7.99, in the mp3 format, but I do not a version of the CD. They list the CD version as being 21.77. Is the U.S. version a download only?

There's no domestic CD release scheduled. It's digital-only here.

The listing shows "Atlantic MOD", made on demand, which for a major label product usually does indicate Amazon exclusivity, but also means that the buyer could receive either a real CD or a CD-R. Maybe some other ICErs would like to test the waters here and report back?
