I sent this Speedball picture to Blake Bell and assumed he didn't know about it because it is not in his checklist, but I think he probably does know but only notes obscure and unknown stories as news on the site, fair enough, he is busy writing us a book.
I have never seen this on any checklist, it's from Official Handbook of the Marvel Universe Update 89#7, inside it says that Joe Rubinstein inks all artwork but I think Ditko did it himself here as the costume has Dr Strange squiggles and not circles as most inkers interpreted it as.
